Has he vet checked the thyroid? That causes hairloss as well

What kind of food are you feeding him? Sometimes a poor diet or the the dogs allergys will cause hair loss and irritated skin which causes them to chew and scratch themselves until bald spots to occur. It might be as simple as changing him to a better food. Some extremely good foods are taste of the wild, and chicken soup for the dog lovers soule. You could also try puting salmon oil on his food. It wil take up to a month before any noticable change.

If you haven't had a Thyroid panel run, have your vet draw some blood and send it off to Michigan State for a full evaluation. Most local labs and vets think testing only the T4 gives a diagnosis, but this isn't enough to pick up the low normals that need treatment.

I would test his thyroid first.

I would be most concerned with a suppressed immune system after having been on Prednisone for so long. Pred can cause many other health problems including kidney failure. Mange sounds possible because it happens when a dogs immune system is suppressed; Which the Pred would cause.

Have a skin scraping done, and bloodwork/urinalysis done.

I would go to another vet to get a second opinion. It is common that vets have different skills and knowledge with animals. There are many sites online that talk about this subject but its hard to know for sure which is your dogs problem.
