Is there any? What's the definition between local and regional metastases?

I have thyroid cancer and my doctor thinks I am having a recurrence and says that they are commonly loco-regional metastases.
I was looking at published work which says there are different recurrence rates for 1. local 2. regional and 3. distant.

Local metastases would be in the same place as the original tumor, regional metastases would be in the same region as your original tumor.
I know that the vast majority of thyroid cancers recur in the cervical lymph nodes, hence the loco-regional metastases statement, and is typically treated with surgery and/ or I-131. http://thyca.org

In general, local refers cancer that is contained within a single organ or tissue type; regional refers to cancer that has spread from the initial site to nearby lymph nodes; and dispersed means anything beyond local or regional.
For cancer in general, recurrence rates tend to be lowest at the local level and higher at the distant level - mainly because the likelihood of missing cancer cells through the initial treatment increases as the cancer cells become more dispersed.


Local cancers can often be removed completely with surgery, and if a cell is missed with surgery, radiation can be concentrated on the entire area where cancer occurs and get any remaining cells.
When cancers are regional, surgery might not get all the cancer cells, and the remaining cells may be too dispersed for radiation to be given in doses high enough to kill all the cancer cells.
With distant cells, radiation and surgery are usually not options at all, and chemotherapy may be the only treatment.
Chemotherapy is usually not as effective as radiation or surgery for killing all the cells since it tends to kill too many normal cells if it is given in doses that are certain to kill all the cancer.
